Send Off & Misconduct Match Reports

Here’s how it works: follow this procedure:

You need the Players Name, Team, & Number. You need to download the Send-off Report from the Library.

You then need to email the form to the Secretary: John Hanlon to john.hanlon55@hotmail.com

You should ring John to let him know there is a report on the way.

Don't forget you can ask John or your mentor how the reports are filled in and completed in the correct manner.
